Services
Our operating companies collectively support tens of thousands of people across the UK, helping them live independently through a range of care support services.
Elderly care
Sooner or later, most of us will need some sort of help to maintain our independence as we grow older. Supporting older people with daily living needs, and personal care in particular, is the bread and butter of most of our businesses. Our care workers travel around local communities helping people over 65 to get up in the mornings, wash, dress, prepare and eat meals and take their medicines.
Our operating companies take pride in providing a flexible and outcomes-focused service. To this end, as with meeting personal care needs, we offer support with practical tasks such as shopping, laundry, housework and managing personal affairs. We also help older people to get out of the house and maintain social contact with family and friends, reducing isolation that can affect older people living in the community.
Care support for younger adults
Across the UK, we support hundreds of younger adults with physical disabilities, mental health issues or learning disabilities, helping them lead as normal lives as possible.
Complex care and live-in services
Some of our operating companies specialise in providing complex care support to people with profound or multiple disabilities. These types of services are typically intensive and may include healthcare and/or nursing support. A number of such services are provided on a round-the-clock or live-in basis.
Support for disabled children
Selected operating companies provide tailored care support for children and young adults with disabilities. Such services range from personal care to support with activities outside the home in assisting our younger clients in having the best start to life they can.
Reablement
Reablement services are increasingly provided to support people who have suffered an accident or sudden deterioration in health to regain essential skills. Properly planned and delivered, good reablement packages can reduce or remove the need for ongoing care support, with great outcomes for clients and customers alike.
Respite Care
Family carers are often under enormous stresses when delivering care themselves, and receiving a little help every so often can be the difference between coping and not coping. We provide a range of bespoke respite services to ensure that family carers receive anything from a few hours to a few days to recharge their batteries and have some time away from the pressure of caring for a loved one.
Extra Care
City and County Healthcare Group is the UK’s largest provider of care support in Extra Care settings. Extra Care is an innovative half-way house between living independently at home and living in a residential unit. Typically, an Extra Care scheme comprises individual private flats rented by tenants.

Innovating through technology
Social care is changing. We know that as people are living longer in your community, there are more pressures on Local Authorities and commissioners. Whether you need to improve services, manage increased demand, or simply deliver more for less – we understand.
That’s why we’ve developed Model Branch – a digital platform that meets these needs. It’s a completely new way of approaching social care. Building on your local knowledge and our expertise in social care, together we can improve lives in your community.
“Now that we’ve gone digital and removed all the paperwork, it means that our carers have more time to spend with our service users.”
– Catherine Porter, Branch Manager
Improving quality
None of this is any help if we’re not giving your community the best care possible. By removing admin and helping everyone see where we can make improvements, Model Branch gives us time and insight to deliver a fully people-centred service.
Together, we can:
- Manage medication and review needs day-to-day
- Update and share care plan data as changes happen
- Increase hands-on time with service users by reducing paperwork
Improving efficiency
The pressure is on everyone to deliver the best services we can as efficiently as possible. Through reporting care delivery and identifying trends as they happen, Model Branch helps us get the most from our service.
Together, we can:
- Optimise care plans by user needs as they change
- Build accurate rostering by service user need and location
- Reduce admin and paper use
Improving visibility
What if you could see how care delivery is performing as it happens? Model Branch gives you complete transparency about where our care workers are, and care we’re providing in your area.
Together, we can:
- Measure accurate, location-based reporting
- Report transparently to everyone involved in care management
- Make informed decisions and manage demands
These changes are already happening in social care. Speak to our team to email us at ModelBranch@candchealthcare.co.uk to find out how we can help you innovate care for people in your community.
